Acquired non tumoral laryngo-tracheal stenosis: Review of 10 years experience

Summary :
Acquired laryngotracheal stenosis are rare but serious with severe morbidities. They are most often due to prolonged endotracheal intubation or laryngo-tracheal external trauma. The treatment is difficult; it’s goal is to restore a respiratory tract while preserving the functions of the larynx. Objective: Describe the epidemiological, diagnostic and management aspects of non-tumor laryngotracheal stenosis at Fann Teaching Hospital. Material et method: We performed a retrospective study covering a period of 11 years, from January 2006 to December 2017 at the ENT department of Fann teaching hospital, Dakar. During this period, 32 patients were hospitalized for management of laryngotracheal stenosis. Résults: There were 17 male and 15 female, the mean age was 28.31 years. Etiologies were dominated by complications of reanimation in 46.87% of cases. The average duration of intubation of 13.26 days with extremes ranging from 6 to 34 days. The location of the stenosis was tracheal in 13 cases (40.62%), larynged in 11 cases (34.33%), laryngo-tracheal in 8 cases (25%). Fourteen patients had endoscopic dilation sessions, twelve patients underwent laser desobstruction during Spanish humanitarian missions and 25 patients underwent external surgical procedure. Conclusion: non-tumor acquired laryngotracheal stenoses are more and more frequent and represent a serious functional handicap. Their treatment is difficult especially in our context.
