Introduction : L'allergie est bien la maladie du siècle. Actuellement en France, plus d'un enfant sur dix est asthmatique et près d'un jeune enfant sur cinq a de l'eczéma. 20 % environ de la population générale souffre de rhinite. L'augmentation de la fréquence des maladies allergiques est rapide et ne peut être expliquée par une simple modification du patrimoine génétique. De nombreux travaux cherchent à établir les raisons de cette hausse. La pollution, le mode de vie, les habitudes de vie sont autant de facteurs actuellement reconnus. D'autres sont évoqués : stress, hygiène, environnement, allaitement... L'asthme, l'eczéma, les rhinites, les rhino-sinusites et les rhino-conjonctivites, principales maladies allergiques sont aujourd'hui de plus en plus fréquentes et posent un vrai problème de santé publique. La connaissance de ces maladies allergiques a pourtant fait des progrès remarquables, tant dans la compréhension des mécanismes physiopathologiques que dans les explorations et les traitements. Le consensus ARIA, en collaboration avec l'OMS, a permis une actualisation des données sur la rhinite allergique et son impact sur l'asthme.
Conclusion : La prise en charge de la maladie allergique doit être globale. Elle passe par une connaissance et une meilleure compréhension de la maladie et de ses traitements et par une prévention optimale, lorsqu'elle est possible, des manifestations allergiques. Le diagnostic doit être précis : l'interrogatoire et les tests cutanés représentent les piliers de cette étape, sans oublier des explorations indispensables en fonction de l'orientation clinique : rhinoscopie antérieure ou fibroscopie nasale, exploration fonctionnelle respiratoire, tests de provocation nasale ... Il est absolument nécessaire d'expliquer au patient et à sa famille, s'il s'agit d'un enfant, la maladie allergique : ses symptômes, ses traitements, son évolution. Les thérapeutiques actuelles, dont l'efficacité n'est plus à démontrer, sont parfois mal utilisées, car leur utilité est mal comprise.
C'est en devenant, en collaboration avec le médecin, acteurs de leur prise en charge que les patients pourront "mieux vivre" avec leur(s) allergie(s).
