Acquired non tumoral laryngo-tracheal stenosis: Review of 10 years experience

Summary : Acquired laryngotracheal stenosis are rare but serious with severe morbidities. They are most often due to prolonged endotracheal intubation or laryngo-tracheal external trauma. The treatment is difficult; it’s goal is to restore a respiratory tract while preserving the functions of the larynx. Objective: Describe the epidemiological, diagnostic and management aspects of non-tumor laryngotracheal stenosis at Fann Teaching Hospital. Material et method: We performed a retrospective study covering a period of 11 years, from January 2006 to December 2017 at the ENT department of Fann teaching hospital, Dakar. During this period, 32 patients were hospitalized for manage­ment of laryngotracheal stenosis. Résults: There were 17 male and 15 fema­le, the mean age was 28.31 years. Etiologies were domina­ted by compli­cations of reani­ma­tion in 46.87% of cases. The average dura­tion of intubation of 13.26 days with extremes ranging from 6 to 34 days. The location of the stenosis was tracheal in 13 cases (40.62%), larynged in 11 cases (34.33%), laryngo-tracheal in 8 cases (25%). Fourteen patients had endoscopic dilation sessions, twelve patients underwent laser desobstruc­tion during Spanish humanitarian missions and 25 patients underwent external surgical procedure. Conclusion: non-tumor acquired laryngotracheal stenoses are more and more frequent and represent a serious functional handicap. Their treatment is difficult especially in our context.
