Les ORL savent désormais que de nombreuses manifestations pathologiques de la sphère dont ils s'occupent sont susceptibles d'être aggravées voire provoquées par un reflux gastro-oesophagien aussi bien chez l'enfant que chez l'adulte qui nous intéresse ici. Laryngites chroniques, granulomes des cordes vocales, pharyngites chroniques, toux chronique, paresthésies pharyngées sont ainsi des manifestations classiquement associées au reflux gastro-oesophagien, même si les études de prévalence ne permettent pas totalement de conclure sur l'augmentation du risque provoqué par le reflux. L'effet néfaste du reflux a été aussi souligné dans les suites de la chirurgie pharyngo-laryngée, dans la survenue des sténoses laryngo-trachéales, dans les sinusites chroniques, dans les otites chroniques et même dans les cancers pharyngo-laryngés.
Toutefois la relation de cause à effet n'est pas toujours facile à établir : si elle est vraisemblable quand il existe un reflux cliniquement symptomatique associé à des éléments ORL évocateurs comme une rougeur de la margelle postérieure du larynx, elle est le plus souvent beaucoup plus hypothétique. En l'absence de symptomes typiques de reflux, le problème qui se pose est de déterminer la place des traitements d'épreuve que prescrit volontiers l'ORL consulté, leur pertinence et leurs modalités. A quel moment devant des symptomes pharyngo-laryngés, l'ORL doit-il prendre l'avis du gastro-entérologue pour l'exploration du reflux (fibroscopie, pHmétrie, manométrie) et son traitement ? Il semble raisonnable d'étayer le diagnostic de reflux lorsque des manifestations ORL potentiellement liées à un reflux persistent malgré un traitement d'épreuve de première intention. Le reflux gastro-oesophagien est donc pour l'ORL un sujet d'un grand intérêt sur lequel ce dossier fournit un point de vue multidisciplinaire.
