Summary :
Bilateral abductor paralysis is a rare presentation of Arnold Chiari type 1 malformation in adults. A number of cases have been described in literature, predominately involving neonates and infants, however, only a handful have been described in adults. We present a case of delayed onset bilateral abductor vocal cord palsy in a 20-year-old male who was admitted to our institution with symptoms of acute upper airway obstruction where a radiological diagnosis of a type I Arnold Chiari malformation was made. We present its symptomatology and review the literature.
