Summary : Background: There is a clinical trend among otolaryngologists and phoniatrians that large, hard and/or asymmetrical vocal fold nodules are managed by microlaryngscopic removal. Others give a trial of voice therapy with uncertain outcomes. Objective: The purpose of this study is to illustrate if the voice therapy is effective in the management of large, hard and/or asymmetrical vocal fold nodules or not to save effort and time and to direct patient to optimal option of management. Methods: This study was conducted on 20 patients with large, hard and/or asymmetrical vocal fold nodules who were subjected to initial voice assessment protocol. Then they received voice therapy (Smith Accent method). Voice assessment protocol was applied after cessation of therapy and after 2 months post therapy. Results were compared together. Results: The results showed that there were significant differences in most of the measured parameters with evident improvement in most cases but there is a need for longer duration of follow up. Highly motivated cooperative candidates give better results. Conclusion: The general outcome of the results supported that voice therapy seemed to be an effective method in the management of large, hard and/or asymmetrical vocal fold nodules. However, further controlled studies using more number of patients and follow up for longer times are still needed to strongly confirm the previous results.
